Health Check

Visa medical health check (including freshmen physical examination): 
 

  1. If you are one of the following, please attend the examination:

    1. Those who need to apply for residence and exit permits in Taiwan (Hong Kong and Macao students, nationals without registered permanent residence in the Taiwan Areas).

    2. Those who are currently holding a Visitor Visa but must change to Resident Visa and then to an Alien Resident Certificate (ARC) card. Resident Visa holder only need to attend freshmen physical examinations.

  2. Time: Please visit the Health Center for more information.  

  3. Venue: NCKU Hospital Outpatient Building 1st Floor Service Counter.

  4. Fees: The preferential price is NT$1300 (MMR three-in-one vaccine costs an extra NT$670).

  5. Required documents:

    1. Passport (Forgin student/ Overseas Chinese Students) / Exit and Entry Permit (Hong Kong and Macao).

    2. One 2-inch photo.

    3. First visit registration form (Please select the Visa Medical Health Check).

    4. National Cheng Kung University for Freshman Physical Examination Consent for Release of Information.

    5. MMR immunisation record. (MMR three-in-one vaccine is needed for those who do not provide immunisation record)

  6. Important reminder:

    1. Blood test is included in the examination, candidates must be fasted for 12 hours.

    2. Please proceed with the physical examination on the scheduled date, to prevent delay in the subsequent re-issuance of the residence permit. If unable to participate in the physical examination on the scheduled date, you must inform and visit OSSS for rescheduling.

 

 

Freshman physical examination: 
 

  1. Candidate: For Resident Visa holder (ex. Malaysia, etc.), ARC holder, Taiwan Residence Certificate with multiple Exit and Entry Permit holder, Taiwan Identity Card holder, only freshman physical examination is needed.

  2. Time: Please visit the Health Center for more information. 

  3. Venue: NCKU Hospital Outpatient Building 1st Floor Service Counter.

  4. Fee: Preferential price NT$850.

  5. Proceed to NCKU Health Centre/Physical examination of freshman, fill up the "National Cheng Kung University for Freshman Physical Examination Consent for Release of Information" and print it. OSSS has coordinated with the hospital in advance, but please make an appointment online.

  6. Required documents:

    1. Passport/ROC/National Health Insurance Card.

    2. One 2-inch photo.

    3. First visit registration form (Please select the Freshman Physical Examination).

    4. National Cheng Kung University for Freshman Physical Examination Consent for Release of Information.

  7. Important reminder:

HIV screening is not provided in physical examination: 
 

The Government of Republic of China (Taiwan) has revised its laws to lift restrictions on entry, stay and residence of non-ROC nationals infected with human immunodeficiency virus (HIV) in addition to removing this item from health examination. The Government of Republic of China (Taiwan) does not offer subsidies to non-ROC nationals infected with HIV infection for treatment in Taiwan. The annual treatment costs for HIV is NTD$300,000 (approximately USD$10,000). It is strongly advised that non-ROC nationals undergo HIV screening in their homeland prior to visiting Taiwan in order to understand their own health conditions. Persons infected with HIV are strongly advised to stay in their homeland for treatment. Persons intending to work in Taiwan are advised to purchase medical health insurance in advance to avoid financial burdens. Upon entry into the Republic of China (Taiwan), foreigners may undergo HIV screening at a hospital to determine their infection status.
